Dr. Jack Blackwell, 79, of Centre, passed away Wednesday, February 6, 2008 at St. Vincent’s Hospital in Birmingham.



Dr. Blackwell was born Feb. 28, to Gumbo and Esther Smith Blackwell. He grew up on Lookout Mountain and graduated in 1946 from Etowah High School. He spent 22 months in the U.S. Navy.

In 1952, Dr. Blackwell graduated from the University of Alabama, and he graduated from the Medical College of Alabama in 1956. After serving an internship at Carraway Methodist Hospital in Birmingham, he moved to Centre, Alabama in 1957 and was in Family Practice for more than 40 years.



He was a charter member and diplomate of the American Academy of Family Practice. For many years, he was a member, Sunday School teacher, and deacon at First Baptist Church of Centre, Alabama.



The funeral service will be held on Saturday, February 9, at 10:30 at First Baptist Church, Centre, the Rev. Jim Wright officiating. Burial will follow in Cherokee Memory Gardens.



Dr. Blackwell is survived by his wife of almost 60 years, Alvatine Bullard Blackwell; daughters, Anne Blackwell, Dr. Linda (Larry) Dolan, Joan (Bill) Perkins, and Beth (Steve) Fowler; grandchildren, Mark, Christy, Jessica, Julie, Nathan, and Sam Perkins; Andrew, Ben, and Sara Beth Fowler; and Elena Dolan. Survivors also include: sisters, Lorene Gladden, Edna (D.L) Russell, Hazel (Harold) Chandler, Marolyn (Max) Cochran, Carolyn (Fred) Shankles, Leola Nelson; and brother, Larry (Sandra) Blackwell. He was preceded in death by three infant children and sisters Mae and Fae Blackwell, and Agnes Reeves.



Special appreciation to the doctors and staff of CVICU at St. Vincent’s for their care and compassion.



The family is requesting no flowers. Memorials may be made to First Baptist Church, Centre, 373 E. Main Street, Centre, Ala. 35960.

The family will receive friends at Perry Funeral Home from 5 to 8 on Friday.
